I agree, by placing my signature below, that I am bound under this contract to participate in league play and playoffs with the team assigned through the duration of my legal league age. I also agree that if I am participating in the GALESBURG JUNIOR FOOTBALL LEAGUE, INC., I will not participate on any other organized football team, I also agree to abide by the rules and regulations of the GALESBURG JUNIOR FOOTBALL LEAGUE, INC., as approved by the Board of Directors. *We, the parents, or guardians, of the above-named candidate for a position on a GALESBURG JUNIOR FOOTBALL LEAGUE, INC. team, hereby give our approval to his/her participating in all GALESBURG JUNIOR FOOTBALL LEAGUE, INC. activities during the current season. We assume all risks and hazards incidental to such participation including transportation to and from the activities; and we do hereby waive, release, absolve, indemnity and agree to hold blameless, the local GALESBURG JUNIOR FOOTBALL LEAGUE, INC. organization, the organizers, board members, coaches, sponsors, supervisors, participants, and persons transporting our child, ward, or organization where games and practices take place.
